Apparently we recieved a form with our reactor pack back on the 23rd of march , which we had to sign saying we had disinfected and rested the isolation facilities for 60 days .
I have no recollection of said form and the now redundant reactor pack has been 'filed'
If i dont send it back within 10 days of today we must undergo a check test within 60 days , and they wont send me out another .
does anyone know how i could get hold of one of these forms , i cant seem to find one on line

Is this an XL vets responsibility now or the ministry? They used to send out a copy if it was needed but I suppose everything has changed

The name of the form is BT05 (cleansing and disinfection) notice. There is not a copy in the XL Farmcare vet area (many other forms) and I can't find the form on the AHPA site for download.
There must be someone else who has been issued one you could get a good quality copy made? Supposedly sent to all reactors.

Well that was silly of the ministry to lose the form you sent back! OR
Never received it? was it sent recorded delivery? do you have proof of postage?

I'm looking at one now - unfortunately filled in. It's actually a BT5 and comes pre printed with a CPHH number, a document reference number and an incident reference number.
I'm guessing you either need to find the original or phone for a replacement and get them to email it.

the spanish ministry vet came out on the 14th april and we did the whole 'new breakdown' procedure , we filled in several forms , but couldnt swear that one of them was a c&d form
come to think of it i couldnt have filled out the form untill the reactor went (one on the 8/4 one on the 20/4 )
we have had two clear tests since , then four days after the second clear (read on the 31/7) had a letter to say the reactor taken on the 8/4 had positive culture result , luckily they accepted two clear tests since her removal both read on severe and said we would have restrictions lifted forthwith .
I have moved over 100 animals off the holding since the 1st august
NOW THEY SAY WE ARE STILL UNDER RESTRICTION DUE TO C&D NON COMPLIANCE
Its less stressful being under restriction than being clear at the moment

We only have (had) 3 pet Highlands now and lost one to tb the other week - amongst all the paperwork thro the post was a disinfection form and a list of approved chemicals - we had a Spanish vet visit us as well who was very helpful.

I had to be reminded too - but only when I questioned my tb status following a clear check test followed by a clear short interval test. Only got away with one short interval on the grounds that the reactor was a
2x ir and had a negative culture.
Give them a ring, jimmer. Explain the situation and email it back asap. We faxed it as well and it was recorded on their system less than two hours later when the duty vet phoned in response to my status request and confirmed we were officially clear.

Have you been paid for the reactor? In Wales they won't pay until the c&d form is in. It's a good incentive.

If this C&D form has been missed, there is no reason on earth that AHVLA / APHA shouldn't send you another. There are many reasons, all human error, by which it may not have been at your farm.
A couple of years ago, our TB10 was not posted out due to a lack of a C&D form. After several attempts to secure TB10 clearance, the conversation went like this.
"We can't issue the TB10, because you haven't completed a BT5"
"We didn't have a BT5, C&D form. The reactor was NVL"
"You need one now" (This was May)
"But you shot the berluddy cow last October"
"The EU have changed the rules. NVL reactors also need a C&D form"
"When did the rules change?"
"Last week".
"So send me one and I'll sign it and send it back"
"Not as simple as that. We need the disinfectant you used and dilution rate"
"So tell me. I'm an OAP now, and memory is shot"
They emailed it, and I scanned it, signed it and sent it back. But all the faffing about cost us a month of the 60 day window to sell.

So did you shift the 100 animals ahead of receiving the official lifting of restriction notice, the TB10? On word of mouth to an anonymous official, after the clear tests?

You shouldn't have moved those animals until you had received in writing the paperwork stating that your officially clear as until then your officially under TB2 and given the amount of animals you have moved ( unless going straight to slaughter ) you may well find yourself in trouble with Trading standards and/or lose quite a bit of your BSP payment!
No reason why they cannot send out another C+D form mind.

I can't 'like' that, but I'm afraid that GUTH is right. It's the paper trail wot counts, and until the TB10 cancels the original restriction notice, then you are still technically under restriction.

Is there no safeguards in place via bcms?
Something should flag up an improper movement.

clear test read on the friday , letter confirming positive culture on the folowing tuesday , a couple of phone calls on wednesday and i had conformation that they had recieved paperwork and test results and that all was fine and we were clear and needed a 6 month test .
quite often been read on the friday and sold on the saturday if clear, same with pmt

so will the reactor pack also contain another c&d form which covers the buildings and equipment used in the previous months to the test when said reactor could have been shedding
